SPECIAL NOTICE: Downloading or viewing Enrollment and Statement of Health Forms

Enrollment and Statement of Health forms open as Zip files, which are multiple documents zipped into one file. Depending on which Internet Explorer (IE) window your computer is using, you may experience a pop up note, in the Explorer window in order for the form to be downloaded/viewed. Internet Explorer 8 users, are experiencing a pop-up window, without the form downloading.

To successfully open and view the forms, follow these instructions:

Quick View (For all IE users): Using your mouse, right click on form link, then click open.

Internet Explorer 8 users: To fix the download issue, open an Internet Explorer window, go to tools, then internet options. Click the Security tab, then Custom level button. Go to “Downloads – Automatic prompting for file downloads” click on enable, then click ok.

Internet Explorer 7 or previous versions: After choosing an Enrollment or Statement of Health form (left click with mouse), a pop-up window will display. Click on the Download File button and a file download box will appear. Click Open to display the form and a temporary file will be loaded on your computer. Click here for additional instructions.
