The Multi-Language Health History Project began as an initiative of the University of the Pacific Dental School (UOP) to address the needs of patients and dentists who do not speak the same language. Each version can be matched against others to allow your dentist to translate from your native language to any of the languages listed. Simply click on the language you speak (or that of a friend or relative) and download the Health History Form. Then download the form in the dentist’s language and bring both versions to the next dental visit! |
